What else should I know?
- Our membership plan is not insurance.
- Payment for any treatment(s) not included in the membership plan is due at the time of service
- The membership runs for 12 months from the date you join
- The membership plan benefits may not be combined with insurance, other offers, or discounts
- The membership plan is priced for existing patients. If you are a new patient, you will be responsible for the cost difference (example — routine exam vs new patient exam)
- The membership plan does not include cash discounts
- The membership plan does not cover the following procedures in our office:
- Cosmetic treatments such as bonding and veneers
- Sleep apnea and TMJ appliances
- Implant Services (CDT codes D6000-D6199), including dentures and partials supported by implants
- Orthodontics such as comprehensive orthodontic treatment and appliance therapy (CDT codes D8000-D8999)
- Our membership plan is not insurance.
